Eligibility for Agency with Choice
To qualify for the Agency With Choice (AWC) model in Massachusetts, an individual must meet the following criteria:
- Be eligible for DDS services
- Be age 18-22 or older
- Live in Southeast Region and be served by a DDS Area Office
- Want greater choice and shared management of staff
