Medically Complex Program
The Nemasket Group Medically Complex Program is a DDS-funded family support service designed to help families caring for a child or young adult with significant medical, physical, and developmental needs remain safely and successfully supported at home. As part of the DDS Family Support system, this program is built on the belief that families are the best natural resource for their child, and that services should supplement and strengthen the family’s own capacity while respecting cultural, social, and economic differences.
This specialized program provides wrap-around, in-home support for children and young adults (typically ages 3–25) who have complex care needs and require coordinated medical and developmental supports. The Medically Complex program focuses on comprehensive case management, helping families navigate multiple systems, integrate medical and community resources, and maintain their child safely at home.

What We Do
Our trained staff partner directly with families in the family home to provide:
- Specialized case management to help coordinate medical providers, in-home nursing, therapies, school services, and community supports
- Service navigation—assisting families in understanding and accessing complex care systems, benefits, and resources
- Information, referral, and emotional support, reinforcing the DDS commitment to family-centered care
- Help integrating the many supports families receive so care feels organized, stable, and manageable at home
- Flexible funding support (as available through DDS) for medically necessary goods, respite, or other supports not covered by insurance
The Goal
The Medically Complex model is to ensure that families have the tools, coordination, and advocacy they need to keep their child at home, promote well-being for the entire family, and help prevent out-of-home placements.
