What is HBCS Waiver Support
We deliver a range of Home and Community-Based Services (HCBS) that keep people living the lives they choose—in their own homes and neighborhoods—with the right supports at the right time. Services are individualized, flexible, and built around the person’s goals.
PBS informs every support plan—proactive, teachable, respectful. We ensure services center on valued social roles and full community inclusion, not just access.

We Offer the Following Services:
Individual Support / Community Habilitation
Individual Support / Community Habilitation provides individualized, in-home or community-based skill building to support greater independence. Coaching may include daily living skills, communication, social skills, budgeting, travel training, and community navigation—always tailored to the person’s goals and preferred routines.
Community-Based Day Supports (CBDS)
CBDS helps individuals build meaningful days through skill development, social engagement, and community participation. Services focus on learning, wellness, volunteering, and developing personal interests—promoting confidence, independence, and valued roles in the community.
Prevocational Services
Prevocational services help individuals prepare for future employment by building work-related habits, soft skills, and practical abilities. Supports may include attendance and punctuality, workplace communication, problem-solving, stamina, and exposure to different job tasks—all designed to help the person move toward competitive employment
Supported Employment
Supported Employment helps individuals find, secure, and maintain competitive, integrated jobs that match their strengths and interests. Services may include job development, job coaching, on-the-job training, and ongoing follow-along support to foster long-term success and workplace inclusion.
Chore Services
Chore Services provide assistance with heavy household tasks that a person cannot safely complete alone. This may include deep cleaning, moving furniture, organizing, or other non-routine home tasks that help maintain a safe, healthy living environment.
Eligibility for HBCS Waiver Support
Adults age 22+
Eligible for services with the Department of Developmental Services, MassAbility through the Home and Community Based Supports Waiver Programs.
Live in the Greater Fall River, Greater New Bedford and Wareham/Onset areas.
